Three self-certification Statutory Paternity Leave and Pay forms have been replaced with a single form.
There are 3 self-certification Statutory Paternity Leave and Pay forms that can be used to apply to the employer. Note that these are not mandatory and employers may have their own versions:
1. SC3 – becoming a birth parent
2. SC4 – becoming an adoptive or parental order parent, and
3. SC5 – adopting a child from abroad
If these are used, they have been replaced with a single 'Ask your employer for Statutory Paternity Pay and/or Paternity Leave’ online form.
Once the employee has completed the form, it should be downloaded and signed before it is sent to the employer. Also note that some of HMRC’s other Statutory Paternity Leave and Pay forms have been removed as they related to a regime in place before Statutory Shared Parental Leave and Pay.
