MLR - any samples of forms filled in (with made up details)

I know I should have been filling in these forms as I took on new clients - but the reality of the situation is I have only three clents at the moment - two are related to me and the other I have known for over 35 years. I've just been putting it off but now realise I must get them done. For the non relative I took a photo of his driving licence and visited his home on several occasions. This was to fill in a Self Assessment return - I will probably spend more time filling in these MLR forms than I did on the SA Return.

Is there any sample of any of these forms already filled in to give me an example of what I should be putting in? For instance, in MLR8 "Services rendered to client" - the box isn't that large - what's meant to go in there? 

I just look at these forms and get then angry at the lack of help on how to fill them in for people new to this sort of thing.

It's not all in the manual - not in the manual I received. Oh yes, all the forms are there but not how to fill them in for someone filling them in for the first time. 

Take the box for  "Briefly explain the reasoning behind your decision to assess the client at this risk level" - how briefly? Bear in mind the size of the box and it has to be filled in with a pen and still be read by someone. As I say, a sample of a completed form (filled in by pen) would take away all this confusion.

Maybe in the future when the forms are filled in online it will take away all this confusion.

Log onto the ICB main site and go to the MLR section. There you will find a heading "How to complete MLR8" It gives good examples of the type of comments to make. It really couldn't be any more helpful.

Thanks - I found it eventually by "searching" the website.....and then found it via the link. I didn't notice the link to start with. I have my browser set to view the ICB website in a larger font and it pushed  the narrow left had column of links off the viewing area. Because the box on the page matched up with the viewing area I wasn't aware of things off to the side. I wonder what else I've been missing out on.
