Communicating AE to Employees

Does anyone have a good source of the various letters which should be communicated to employees?  Examples being the standard opted in for eligible, not opted in but you can for non eligible and entitled, postponed, etc.


I had a really good letter for one client with calculated examples of what the deductions (ee and er) would be, but can't recall where from nor find anything nearly as complete again.
